Toxicidade da Amiodarona

Resumo

Os autores descrevem o caso de um doente do sexo masculino, de 49 anos de idade, com fibrilação auricular permanente diagnosticada há dois anos e medicado desde então com amiodarona (200 mg/dia), que foi internado por quadro de astenia, anorexia e emagrecimento acentuados, acompanhado posteriormente de icterícia, rouquidão e trémulo dos membros superiores. Após estudo exaustivo foi feito o diagnóstico provável de toxicidade múltipla da amiodarona (tiróideia, hepática, cutânea, ocular). É importante salientar neste caso, a presença dos múltiplos efeitos secundários da amiodarona num doente medicado com doses baixas deste fármaco, durante um curto período de tempo. Pretende-se também alertar para a necessidade do uso da amiodarona apenas quando estritamente necessário e com a monitorização adequada.

Abstract

Amiodarone Toxicity

The authors describe the case of a 49-year-old male patient with permanent atrial fibrillation diagnosed two years ago, being medicated with amiodarone (200 mg/day). He was hospitalized with clinical manifestations of asthenia, anorexia and emaciation, later accompanied by jaundice, hoarseness and tremor of the upper limbs. It was made the probable diagnosis of multiple toxicity of amiodarone (thyroid, hepatic, cutaneous, ocular), after an exhaustive examination. It is important to pay particular attention to the presence of multiple side-effects of the amiodarone in a patient treated with low doses of this medicine during a short period of time. This is one of the reasons why amiodarone should only be used when extremely necessary and under medical supervision.
